FISA spying

Napolitano: How the US government can spy on you http://video.foxnews.com/v/5728353968001/ Judge Nap says: "If you want to buy a book from a book shop in London England or Florence Italy and you make these two phone calls the government's computers will be alerted immediately that someone in the United States has called a number that they don't recognize in a foreign country. They will download a digital recording of the phone call..." I don't want to get into the politics of this; my question is how does the NSA listen in on these types of calls? I assume they have cooperation with folks like AT&T, Verizon, etc but what about over VOIP? This doesn't personally affect me. I never call anyone outside the US. I'm just curious from a VOIP perspective how this is done.
