Pbxes.com now supports GV-SIP and is included with "Free" accounts. There is now a one-time $15 charge for new free accounts. Existing free accounts are still totally free. I learned about Pbxes GV from SteveInUS.

Click on Personal Data and enter your password. You don't have to confirm. Click on Submit and Start.
To define GV click on Trunks then Add SIP Trunk. Click Get Password for Google Voice. The Google Login page will be displayed. Login and click Allow Access. All the necessary info will be automatically defined. Click Submit then click on the red bar that appears on the top of the page. Do not change the Trunk Name.
You will also have to define an Extension and In/Outbound routes.

They had several problems, but the only open problem is no ringback when calling some numbers. This only happens for me when calling another GV number. I switched from Raspian/GV-SIP, too many SDHC cards failing.
