[Voip.ms] VOIP.MS sound issues

We have been VOIP.MS customers for many years and although our configuration has not changed, our outgoing sound quality (what the people we call hear) is terrible. It is also bad when we call 4443 their echo service. 60% of the time the sound is great with little latency, but 40 % of the time it's nothing or jumbled nonsense. It is worse when we call using an android Softphone (CSIPSIMPLE). We have cable internet, a Thompson DCM475 modem router, an ASUS RT-66 wifi router connected with a cable to a CISCO 122. The issue with the softphone is the same from any wifi location. Thanks for insight on what may be wrong. Maybe carriers are dropping on purpose VOIP packets on purpose?
